What are self-directed services?

Self-direction, also called consumer-direction, is a way for people who need long-term care to receive support and stay in their homes and communities. Consumer Direct Care Network offers self-directed services through a variety of self-directed programs across the country.

If you are eligible to be supported through a Medicaid waiver program, you may qualify for self-directed services. As a Financial Management Services (FMS) provider, we can support you in managing your care.

Ang self-direction ay tungkol sa personal na pagpili, kontrol, at flexibility.

Paano Gumagana ang Self-Direction?  

Sa sariling direksyon, ikaw (o isang taong pinagkakatiwalaan mong gagawa ng mga pagpipilian para sa iyo) ay pipiliin at sanayin ang mga taong nagmamalasakit sa iyo, magplano kung kailan sila tutulong, at piliin kung paano ka nila tutulungan. Sa mga self-directed na serbisyo:

  • Manatili ka sa iyong tahanan at komunidad.
  • Pinipili mo ang iyong mga tagapag-alaga, maaaring maging mga kaibigan o pamilya.
  • Pamahalaan mo ang iyong pangangalaga. Ikaw na ang bahala.
Mag-hire o pumili ng sarili mong tagapag-alaga na susuportahan ka sa iyong mga pangangailangan sa pangangalaga.

Iba pang Serbisyo

Caregiver training provides prospective and current caregivers with the required training needed to provide care.

Personal Emergency Response Systems help Montanans maintain independence with medical alert services 24 hours a day, 365 days a year.

Respite services provide relief to your primary unpaid caregiver while ensuring your needs are met. Your respite worker can provide personal care, support, supervision, and companionship.

Vendor Goods and Services provides support through the vendor payment process, helping you purchase medical and in-home care products and services and stay within your program budget.

Veteran Directed Care supports independence so Veterans can remain at home.
