A support broker is a resource provided by some Medicaid waiver programs. Our support brokers work with you to coordinate and manage your self-directed services. We value your strengths and abilities and will be your partner in creating the more independent, self-directed lifestyle you desire.

How does my support broker help me?

A support broker can help with creating and updating service plans and emergency/backup plans, understanding paperwork, offering information on community services, and answering questions or concerns about directing your own care. Support brokers also may assist with:

  • Caregiver recruitment, paperwork, and management
  • Budget activity monitoring
  • Short- or long-term support
  • Timesheet review and submission
  • Housing counseling
  • Job application assistance and interview prep
  • Coordination of independent living activities
